Make a Report

This form is used for anyone wishing to make a report regarding a violation of the MHU Title IX Policy on Sex- and Gender-Based Harassment and Discrimination. As a reminder, all MHU employees (excluding Confidential Resources) are considered Responsible Employees and must report allegations of a Title IX Policy violation.

*Please note this form does not constitute a Formal Complaint as described in the Title IX Policy. Formal Complaints must be completed, signed, and sent via email, mail, or delivered in-person to the Title IX Office.
