Confidentiality and Outgoing Referrals

Outgoing Referrals 

Disability Services may include referring a student to additional services within the university system or to other private or community services. The Disability Services Coordinator may assist a student with accessing these services by offering recommendations, contact information, and/or completing referral documents for/with the student. Copies of referral documents and documentation of referrals being made with/for the student are kept in the student’s Disability Services case file. When appropriate, the Authorization for Use and Disclosure of Protected Information form is completed, signed and dated by the student and submitted to the referred services provider. A copy of this form is included in the student’s Disability Service case file.


Every effort is made to protect the privacy of students seeking services through Disability Services.  Information shared is limited only to MHU personnel whose assistance is required to implement requested accommodations. The Disability Services Coordinator will seek written permission from the student receiving services prior to discussion of any service with anyone other than the student's instructors, the Director and Assistant Director of Residence Life in the case of housing accommodation requests, and/or members of the Disability Services Committee.
